30 JANUARY/FEBRUARY 2018 • FOGHORN SAFETYMATTERS The working group conducted a comprehensive review and evaluation of five years of Coast Guard casualty data specifically focused on slips, trips and falls. They reviewed the existing PVA DVD and manual on slips, trips and falls, and solicited additional data from vessel operators. The working group produced the following: A White Paper on the current state of the industry, casualties, near misses and the impact slips, trips and falls has on the industry. The paper also provided recommendations to the Coast Guard and the Passenger Vessel Association on how to reduce incidents; and A FOGHORN article (August 2017) that discusses results and mitigation strategies. The information in the White Paper is currently being used to update the PV A Slips, Trips and Falls manual. As a result of the working group, the Coast Guard is changing how they collect data and are requiring more information from casualty in- vestigators in the field. The Coast Guard reported that cases are getting kicked back for lack of specific infor- mation and that will improve data quality. Changes in the 2629 reporting form are also helping to improve the quality of investigations. Small Passenger Vessel Automation Also Chartered in May 2015, this working group was tasked to update inspection and plan review guidance for Coast Guard field units and the Marine Safety Center regarding small passenger vessel electronic propulsion control systems; required versus dis- cretionary testing of machinery alarms if installed; and testing of required alarms and trips on prime movers with Electronic Control Modules (ECMs) for engine management. Captain Mike McElroy with Wendella Boats led the PVA effort on the working group and was supported by PVA Associate members in the engine and engine control fields. Over the last two years, the working group reviewed existing guidance and provided recommendations to Coast Guard Headquarters and the Marine Safety Center on plan review and in- spection protocols for automated systems aboard small passenger vessels. As a result, the Coast Guard has updated plan review guidance to establish clear lanes of responsibil- ity for owners, manufacturers, the Marine Safety Center and field in- spection personnel when dealing with automated systems on SPVs.
